Feedback forms are a collection of questions that you may want to use when you send out a feedback campaign.
To create a feedback form, navigate to Admin > Feedback > Forms and press
- Give the form a name, and make it recognisable and specific so that it is easy to find the right form when sending a campaign.
- When you want to make it available to use in a campaign, turn on 'Published' using the toggle. If you want to keep it as a draft make sure the toggle is set to off.
- Add form items - you can have as many as you like.
Tip! - This is a toggle!
Section Headings - these appear above the question that the user sees and are great for categorising questions.
Rating Question - there are a number of different rating options to match the question. You can see them here. With all of the rating questions, you can also add an evidence box if you want respondents to expand on their ratings.
Tip! On the summary reports for the admin and the user if you choose to use either Numbers or Stars throughout your form you will see average scores. This will not work if you chose to have different rating options - it is only applicable for Numbers or Stars.
Multiple Choice Question - Here you can add your own options and the user can select one. Perfect for simple yes/no questions.
Free Text Question - This can either be single line answers or multiple lines and is great for getting some qualitative feedback.
Rich Content - This is perfect to use at the start and end of the feedback form for introductions, to give context and to provide helpful hints to the user who is about to respond to the questions.
Once you have added all of your questions it is super easy to view what the respondents will see by pressing and, if you are happy, go ahead and save.
This form will now be in your Forms List and available to use in campaigns provided you have clicked on 'Published'.
Tip! If you have already created a form and you want to use the majority of the questions again in another form, in the Forms List press
to duplicate. Now you can edit that form without having to re-write all of the questions!
